Blade Vs Blade-Free Laser Eye Surgery

Eye Injuries and Children

Children can get into all kinds of scrapes and situations, but one of the most terrifying kinds of injuries involves those that affect the eyes. Several types of injuries and conditions can traumatize or otherwise endanger children's eyes.

Environmental Irritants
The accidental introduction of an environmental irritant, such as dust, soap, or other particle, to the eye can cause pain, redness, and corneal abrasions.

Using water from the home tap to flush out the irritant represents the easiest and most effective way to deal with this issue. Use only lukewarm water, never hot, when flushing out the foreign body.

Never attempt to wipe the object off the eye with a towel or napkin. This could irritate, abrade, or damage the cornea and cause a more serious eye problem.

A more serious eye injury occurs when something penetrates the eye. Initially, cover the area with a cup to prevent anything else from irritating the eye and potentially causing an eye infection. While waiting to see the doctor, applying a cold cloth to the affected area can relieve some pain and swelling. Create a cold cloth by wrapping a wet towel or washrag around a couple of ice cubes. Never apply the ice directly over the affected eye. Watch out for continuous eye pain, increasing redness, vision changes or a change in the appearance of the eyeball.

Giving your child acetaminophen may relieve some of the pain, but avoid ibuprofen or aspirin as these can exacerbate bleeding, if any.

Chemical Irritation
If any household chemical should get in contact with your child's eyes, flush the eyes with lukewarm water for a minimum five full minutes, then seek medical assistance to avoid eye infection or damage. The attending pediatrician can help your child more quickly and efficiently if you can provide the name of the chemical, or even bring a sample with you when you go to the emergency room.

First Aid
Knowing how to properly handle an eye emergency can save valuable time and possibly prevent vision loss.

Basic Eye Safety and Preparedness:
  • Wear eye protection for all hazardous activities and sports at school, home and on the job.
  • Stock a first aid kit with a rigid eye shield and commercial eyewash before an eye injury happens.
  • DO NOT assume that you know the severity of any eye injury. When in doubt, see a pediatric physician immediately.
Eye Contact with Chemicals:
  • Immediately flush the eye with water. Hold the eye under a faucet or shower, or pour water into the eye using a clean container. Keep the eye open and as wide as possible while flushing. Continue flushing for at least 15 minutes. DO NOT use an eyecup.
  • If a contact lens is in the eye, begin flushing over the lens immediately. This may wash away the lens.
  • DO NOT bandage the eye.
  • Seek immediate medical treatment after flushing.
Specks in the Eye:
  • DO NOT rub the eye.
  • Try to let tears wash the speck out or use eyewash.
  • Try lifting the upper eyelid outward and down over the lower lid.
  • If the speck does not wash out, keep the eye closed, bandage it lightly and see a doctor.
Trauma:
  • Apply a cold compress without putting pressure on the eye. Crushed ice in a plastic bag can be taped to the forehead to rest gently on the injured eye.
  • In cases of pain, reduced vision or discoloration (black eye), seek emergency medical care. Any of these symptoms could mean internal eye damage.
Cuts and Punctures:
  • DO NOT wash out the eye with water or any other liquid.
  • DO NOT try to remove an object that is stuck in the eye.
  • Cover the eye with a rigid shield without applying pressure. The bottom half of a paper cup can be used.
  • See a doctor at once!

Vision Loss Among Older People

Most commonly, vision loss among older people is due to colluding of the lens of the eye (cataracts) or to damage of the optic nerve (as occurs in glaucoma) or the retina (as occurs in age related macular degeneration and diabetic retinopathy). A less common cause of vision loss is blockage of the blood supply to the eye. Eyelid disorders mostly change the appearance of the eye, but they can cause discomfort and contribute to vision loss as well.

Whatever the reason for vision loss, any vision change can compromise an older person's quality of life and, indirectly, health. Loss or vision can be especially devastating to older people coping with other problems as well, such as poor balance and hearing loss. In such cases, vision loss can contribute to significant injury and can impair a person's ability to perform daily activities.

Eye Conditions:
Normal age-related vision loss is called presbyopia. Presbyopia is caused by hardening of the lens of the eye and usually begins age 40. Presbyopia isn't a disease but rather a natural aging process, and diagnosed by a simple examination. Although presbyopia is a naturally occurring process rather than a disease, a number of diseases do cause vision changes that have different symptoms and treatments.

Cataract is a clouding of the lens of the eye. A cloud lens blocks light from entering the eye. Most cataracts grow slowly until they cloud the entire lens, causing progressive, painless vision loss. Cataracts are the most common cause of reversible vision loss among older people in the United States. The first symptom of a cataract is often blurred vision. Sometimes glare is the initial symptom. Colors may seem more yellow and less vibrant. The person may see rings of light around objects (double vision).

Glaucoma is classified as open-angle (chronic) or closed angle (acute). Open-angle glaucoma is much more common than closed-angle glaucoma. In both kinds, people who have had glaucoma in one eye are likely to develop glaucoma in the other eye. Either kind can cause blindness if left untreated. Blindness often can be prevented, however, with the use of eye drops and surgery that decrease eye pressure.

Open-angle glaucoma is painless and causes no early symptoms. The most important symptom is the development of blind spots, or areas of vision loss, over months to years. The blind spot slowly grow larger and merge. The changes may progress to the point where only a small central island of vision remains, in which the person can see straight ahead but becomes blind in all other directions (tunnel vision). If glaucoma is left untreated, eventually even central vision can be lost.
Closed-angle glaucoma causes abrupt onset of severe pain in and around the eye, headache, redness, blurred vision, rainbow colored halos around light, and sudden vision loss. Close-angle glaucoma is considered a medical emergency, because if left untreated, blindness can occur quickly as 2 to 3 hours after the start of symptoms. The goal of treatment is to prevent the onset of vision loss or to stop its progression. For Open-angle glaucoma; drops are the primary treatment. They either decrease production or increase drainage of fluid in the eye. An older person may find it difficult or impossible to administer eye drops because of arthritis or other impairments. Age- Related Muscular Degeneration: Deterioration of the macula, the central and most sensitive area of the retina, Macular degeneration is common cause of irreversible central vision loss among older people. Muscular degeneration is categorized into two types:
Dry (non-neovascular). The most common, the light-sensitive cells of the macula may be lost, and the retina thins (atrophies). Wet (neovascular). Abnormal blood vessels develop beneath the macula. These vessels leak fluid and blood, which result in the formation of scar tissue. Scar tissue can impair vision by causing light-sensitive cells to die. Only a few people with dry muscular degeneration eventually develop wet macular degeneration. However, once wet macular degeneration develops in one eye, there is a 50%chance that it will develop in the other eye within 5 years. People who smoke or who have high blood pressure may be at higher risk of progression to wet type. Most people with dry type have no symptoms, although some people experience mild distortion in their vision. People with the wet type often experience an abrupt deterioration and distortion of vision over days or weeks.

People who experience distortion in their central vision or the gradual appearance of a blind spot on one eye, from any type degeneration, have difficulty reading, driving, seeing faces, and watching television. People retain their peripheral vision, which allows them to perform daily tasks (other than driving and reading) and remain independent. Total blindness is rare, but if both eyes are affected, blindness may result.

Diabetic retinopathy is Damage to the retina that results from diabetes mellitus. This is one of the most common causes of blindness in older people in developed countries. Diabetic retinopathy is most common in people with a history of poorly controlled blood sugar levels. The risk is especially high in those who have had diabetes for many years.

Every patient will need follow up consultations and check ups, but these should be included in the treatment costs. If continued check ups are needed then a fee may be applicable, but normally only a few check ups over the following months are needed.

Prices can be different when it comes to correcting refractive problems such as long or shortsightedness. As with the treatments above, only guidelines can be given as it varies between hospitals and what treatment is needed. Eye Injuries – Causes of Eye Injuries

Eye injuries range from the very minor, such as getting soap in your eye, to the catastrophic, resulting in permanent loss of vision. A black eye is often a minor injury, but it can also appear when there is significant eye injury or head trauma. A visit to the doctor or an eye specialist may be required to rule out serious injury, particularly if you're not certain of the cause of the black eye.

A large abrasion may take a long time to heal, and can cause an inflammatory reaction within the eyeball. Sometimes a sharp object cuts into the deep tissue of the cornea. This can permanently change your vision. Debris in the scratch, such as shreds of tree bark, can lead to infection and ulceration of the cornea.

Blunt trauma occurs when something hits you in the eye. Blunt trauma causes most sports-related eye injuries. Some serious examples are an orbital blowout fracture (a broken bone under the eyeball), a ruptured globe (broken eyeball) and a detached retina (the part of the eye that is sensitive to light and helps you see). Bruising of the eye and eyelid (a "black eye") looks bad but usually is a less serious injury.

Causes of Eye Injuries

Common causes of eye injury include domestic or industrial accidents (for instance, from using a hammer or liquid chemicals or cleaners), assault, car battery explosions, sports injuries (including air-gun or paint pellet-gun injuries), and motor vehicle collisions (including air-bag injuries). Exposure to strong ultraviolet light, as from a welding arc or bright sunlight reflected off snow, can injure the transparent dome on the front surface of the eye (cornea)

Chemical exposures and burns: Often the result of a splash of liquid getting in the eye, a chemical burn can occur in a number of ways. Many chemicals, such as soap, sunscreen, and even tear gas, are primarily irritants to the eye and do not usually cause permanent damage. However, acids and alkalis are highly caustic and can cause severe damage.

Bright sunlight (especially when the sun is reflecting off snow or water) can burn your eyes if you do not wear sunglasses that filter out ultraviolet (UV) light. Eyes that are not protected by a mask can be burned by exposure to the high-intensity light of a welder's equipment (torch or arc). The eyes also may be injured by other bright lights, such as from tanning booths or sunlamps.

Internal pressure in your eye, called intraocular pressure, allows your eye to hold its shape and function properly. Intraocular pressure is like air in a balloon — too much pressure inside the balloon affects its shape and may even cause it to pop. In the case of your eye, too much pressure can damage the optic nerve.

Power tools, lawn mowers, trimmers and weed whackers all pose potential hazards to your eyes. Be sure that all of your power tools are in good condition and only operate them with the safety features engaged. You should also make sure that any rocks and debris are cleared from your lawn before mowing (so they don't get propelled into your face while cutting), and wear protective goggles when using any power equipment to shield your eyes from dust, debris, sparks, fumes and more.

Three Common Age Related Eye Conditions that are Easily Treatable


For all age groups, there are eye conditions that can range from inconvenient to sight threatening, but it is the older generation that tends to suffer the most from sight impairment. That is somewhat understandable, particularly since the eyes are used so intensely over such a long period of time.

Many of the 50 years and over age group face some of the most serious conditions, most of which require eye surgery, whether laser or traditional. But while the list can be long, there are three in particular that are quite commonly developed in old age. These are diabetic retinopathy, cataracts and wet and dry macular degeneration, treatment for each is easily available.

Macular Degeneration

Most of those who suffer from macular degeneration are aged over 50 and, within this group, sight loss is most commonly attributed to this eye disease. The macula is at the centre of the retina and allows the eye to see more sharply the details in our vision

As the name of the condition suggests, the macula in the eye degenerates over the years, until finally the detail in our sight is lost leaving objects distorted and blurred. Think of the reading chart that people must read when opticians are assessing the strength of their prescription lenses. As he or she reads downward, as the words get smaller, they blur. Normally, the younger the person, the lower down the list they can read, while the older they are the higher their vision will begin to fuzz up and the words become illegible.

There are two types of the condition, however, with the dry form accounting for 90 per cent of cases, and wet macular degeneration the remainder. Dry cases develop slowly over a long period of time and can be operated on.

Wet cases, meanwhile, are rare and develop over a much shorter period of time. Abnormal blood vessels develop and leak, damaging the retina on the eye wall, causing severe swelling. These are usually treated with injections of highly effective anti VEGF drugs into the eye.

Diabetic Retinopathy

In truth, anyone with diabetes can have conditions of the eye, but many people who were not diagnosed with diabetes when young can go on to develop the disease later in life. Statistically, diabetics are more likely to develop glaucoma or cataracts than non diabetics, but diabetes can also affect the retina of the eye quite severely.

The first stage of the condition is referred to as background diabetic retinopathy and involves the development of small haemorrhages that look like small dots. However, sight is not in danger at this early stage.

The second stage is called diabetic maculopathy, which involves leaking blood vessels that swell the retina. This particular type is common in Type 2 diabetes and can affect the 50 years plus age bracket. The condition can progress quickly, which means that sight is at risk if it is not treated.

Another stage to the disease is proliferative diabetic retinopathy, which is when haemorrhaging can leak into the vitreous gel in the middle of the eye. This is caused when abnormal vessels on the retina are grown in response to a shortage of oxygen, but which end up bleeding into the eye. It is common amongst Type 1 diabetics. Should bleeding lead to scarring then the scar tissue can damage the retina and lead to complete blindness.

Treatment for all of these conditions can vary, depending on their severity, and it is normal to have eye tests carried out, including OCT retinal scanning, before deciding on the course of action. Lasers are used to treat second stage symptoms, with steroid injections another option. Laser burns are also applied to proliferative diabetic retinopathy. If there is bleeding in the vitreous gel, then a vitrectomy is necessary, where the bloodied gel is removed.

Cataracts

This condition is very common, with around 66 per cent of people over 60 suffering from cataracts. As eye conditions go, these are more inconvenient than sight threatening, with the chief symptom being clouded vision, though it can progress to blindness.

While the early stage of the condition can be treated with glasses, the more advanced it becomes the more necessary eye surgery becomes. Cataract surgery involves using an ultra sound to break up the lens that has clouded over, and then replaced by a permanent artificial lens. The process is quick and easy, taking only about 20 minutes on each eye, almost as simple as dry macular degeneration treatment.

This is a common, everyday complaint at many ophthalmologists and vision centers. The fact that so many people are spending less time outside in natural sunlight, especially in the winter months, is contributing to this growing problem of sore, computer eyes. It has been given the name "computer vision syndrome" and ophthalmologists are warning people to take precautions to avoid this eye syndrome that can lead to further eye ailments.

Computer monitors are designed with lighting that is very unnatural to the human eye. The black-on-white print is also hard on the eyes and when you stare at a computer for hours, it greatly strains the eyes. Our eyes are not meant to stare at unnatural light for hours, let alone on a daily basis. People who work on computers all day are finding this to be causing a threat to their jobs.

But you experience the extra tissue as near or far sightedness, or astigmatism. LASIK surgery is designed to “ablate,” or clean, the extra tissue from the cornea, allowing it to revert to its natural healthy state.

Everyone who has near or farsightedness knows what it means. But not many people can accurately describe astigmatism. Astigmatisms also affect the corneal light refraction, but they cause visuals symptoms like starbursts, haloes, decreased night vision, and ghost images.

* The third type of detachment happens when fluid collects underneath the layers of the retina, causing it to separate from the back wall of the eye. This type usually occurs in conjunction with another disease affecting the eye that causes swelling or bleeding.

Symptoms of a retinal tear
If you have a tear, you may notice : -

* Floaters (specks or threads in your vision).
* Flashes (lights, stars or streaks in your vision).
* Sudden blurry vision.

Symptoms of a detachment

The most common symptom is a shadow spreading across the vision of one eye. You may also experience bright flashes of light and/or showers of dark spots called floaters. These symptoms are never painful.

Many people experience flashes or floaters and these are not necessarily a cause for alarm. However, if they are severe and seem to be getting worse and you are losing vision, then you should seek medical advice. Prompt treatment can often minimise the damage to your eye.

Signs and Symptoms of Retinal Detachment

* Light flashes
* "Wavy" or "watery" vision
* Veil or curtain obstructing vision

* Shower of floaters that resemble spots, bugs, or spider webs
* Sudden decrease of vision

How to Prevent Vision Loss

Early treatment helps prevent vision loss from retinal tears and detachment. If you have any of these symptoms, immediately consult an eye doctor.

You are at greater risk for tears and detachment if you : -

* Are very near-sighted.
* Have had a detachment before or
* Have a family history of retinal problems.


Prevention

If your family has a history of retinal detachment and your doctor finds a weakness in your retina, then preventive laser or freezing treatment may be advised. However, in most cases it is not possible to take preventive action.

Retinal detachment does not happen as a result of straining your eyes, bending or heavy lifting.


Fixing a detachment: Detachments can be treated in the doctor's clinic or a surgery centre. You may have more than one type of treatment.

They include the following : -

* The tear that caused the detachment is sealed with a laser or by freezing.
* The vitreous may be removed by surgery to keep it from pulling on the retina.
* A gas bubble may be placed in the eye. Or a flexible band may be placed around the eye. These hold the retina in place.



Treatment

There are a number of ways to treat retinal detachment. Your retinal surgeon will determine which treatment is best for you based on the type, severity, and location of the detachment.


Pneumatic Retinopexy

In this procedure, the retina surgeon numbs the eye with local anesthesia and injects a small gas bubble into the eye's cavity. The bubble facilitates the repositioning of the retina back into its normal location, and keeps the two detached layers up against each other. Because the gas rises, this treatment works best for detachments in the upper portion of the eye.

Over a period of one to two weeks, the eye gradually absorbs the gas bubble. During that time, your eye surgeon will perform another procedure at our Florida practice to "tack down" the retina. This can be accomplished with a laser or through cryotherapy.


Scleral Buckle

If you have a detached retina of a certain size and location, your retina surgeon at our Tampa Bay area center may treat it with a scleral buckle. When this technique is used, a tiny sponge or band made of silicone is secured around the eyeball, pressing inward and holding the retina in position. It is placed beneath the eyelids so it will not be visible after surgery.

After placing the scleral buckle, your retina surgeon may perform a surgical procedure called a vitrectomy to remove the vitreous gel from the eye, and may also seal a few areas of the retina into position with laser or cryotherapy. The scleral buckle technique may elongate the eye and cause nearsightedness, which may require post-operative glasses.



Reattachment with Silicone Oil

When other retinal detachment surgeries prove ineffective, silicone oil may be used to reattach the retina. To perform this procedure, the eye surgeon removes the vitreous gel and replaces it with silicone oil. Vision is extremely poor when the oil is inside of the eye. After the retina has resealed itself against the back of the eye, a second procedure is often performed to remove the oil.

After the operation

Some people will be encouraged to get up and carry on as usual on the day after the operation, although most people will be asked to keep their head in a particular position to help the healing process. Your eye specialist will prescribe eye drops and you will need to use these for a few weeks.

You can resume normal activities, including sex, as soon as you feel able.
